My horse throws his head around all the time, every direction when riding?
I need a martingale to fix this.

he is only 5 so he needs teaching that throwing his head around like a crazy fool, in every direction is not right
His teeth have been checked and his tack is fine... i just don't get this
I am not really holding the reins tight at all.

I had a thoroughbred gelding that took a bit just fine in the beginning of his training, but after one long trail ride, he refused to take a bit ever again. He also would throw his head, and even smacked me in the face once while riding.
Your horse may just be one of those that does not like a bit. Even though his teeth are fine, a bit may be uncomfortable, or too severe. If going to a milder bit doesn't help, your horse may just be entertaining himself by tossing the bit in his mouth, or trying to get his tounge over it.
You could try a "bitless bridle" or a "stop and turn" , I put my gelding in a hackamore, and he settled right down, none of these are invasive, which will allow him to concentrate on your cues rather than whats in his mouth.
In any case, the longer he does this, the more likely it will become a habit, taking more time to correct.
